Expandable activity boxes

Makes command output and file edits inspectable directly in the conversation. Expand a command to see its output, exit status, and duration, or expand a file change to review its diff. Activity stays boring while collapsed, and updates from a running command merge into the same entry.

The missing output came from the projection pruning superseded command rows before their completion carried every chunk. I made the merge keep the longer snapshot and covered it with tests.
